Sprint Retro Survey

This survey is designed to collect structured feedback from Sprint Retrospective sessions, focusing on team behaviors, reflection quality, and follow-through on improvement actions. It helps agile coaches and facilitators identify patterns in how teams inspect their work, adapt their processes, and foster psychological safety.

Once submitted, the results of this survey will be sent to the interviewer’s email address, and—if a Summary Recipient is selected—shared with them as well. Respondents will also receive a maturity score ranging from 1 to 100, which can be used to benchmark current practices and track progress over time.  

The Additional Notes and Critical Feedback fields will not be included in the Summary Recipient’s e-mail.  These are notes for the interviewer only.
