This survey is designed to collect essential background information about a team, including its members, roles, geographic distribution, focus areas, and key business stakeholders. It helps agile coaches and transformation leaders build a clear picture of the team’s operating environment, strategic alignment, and decision-making structure.

Once submitted, the results of this survey will be sent to the interviewer’s email address, and—if a Summary Recipient is selected—shared with them as well. Respondents will also receive a maturity score ranging from 1 to 100, which can be used to benchmark current practices and track progress over time.  

The Additional Notes and Critical Feedback fields will not be included in the Summary Recipient’s e-mail.  These are notes for the interviewer only.
